Southern Regional is the only hospital in south metro Atlanta that has
an innovative, surgical care coordinator program to help guide patients
through the entire surgical experience.
We understand that surgery can make patients and their loved ones nervous,
so our care coordinators are here to help make your experience as easy
as possible. We are committed to providing the best care before, during
and after surgery, and our care coordinators play an important role in this.
Here's how it works. Each patient is assigned a care coordinator who
walks them through every step in the surgery process, from pre-assessment
to post-operative care.
From admission to discharge, care coordinators act as the single point
of contact for patients and the entire medical team devoted to their care.
Prior to surgery, they educate each patient about what to expect of the
upcoming procedure and arrange for any pre-surgery tests. They also provide
support on the day of surgery and keep waiting family members informed
After surgery, care coordinators visit every inpatient. They help facilitate
communication with the physician and care team, review any post-operative
care needs and help with discharge. And they don't stop there. Care
coordinators continue to support patients after discharge, including follow-up
care or any help needed for a complete recovery.
