What is a Patient Portal?

Southern Regional Medical Center's Patient Portal is a secure website that offers our patients 24-hour access to their personal health information from anywhere with an Internet connection. It's easy to set up your patient portal account

  • Click on the "Patient Portal" link below
  • Screen will pop-up and select the "Sign Up" button
  • You will be requested to submit the following information:
    • Last Name / First Name
    • Birthdate
    • Gender
    • Medical Record Number or last 4 Digits of Social Security Number
    • Email Address (and confrim email address)
      • You MUST enter the email address that you previously provided Southern Regional (usually done at registration).
  • You will then receive an email with a one-time use login and password.
  • Log in and choose a unique (secure) username and password.
  • Once you have enrolled, you will be able to view your medical record.
